Summary:
Provide clinical insight to patients enrolled in 4myheart Centers incorporating a disease management focus as associated with Berkeley tests with the anticipated goal of reducing risks for cardiovascular disease and stroke. Participate in ongoing communication with physicians regarding progress and/or potential concerns related to improvement, or lack thereof, in cardiovascular risk markers associated with Vxs (repeat testing to measure progress)
Essential Duties and Responsibilities:
- "Coach" patients in an ongoing educational process to include but not limited to the following:
- Test result overview (after patient review with physician) and its association with disease process and risk
- 4myheart utilization
- Medications and their relationship to improvement in cardiovascular risk markers
- Potential side effects to medications and tips as to ensure success in compliance
- Importance of monitoring progress through "report card" of Vxs
- Nutritional counseling as related to test results and co-morbidities (ie HTN, DM, CAD, etc.) and lifestyle
- Exercise counseling as related to test results, co-morbidities, age, and lifestyle
- Identification of potential barriers (socioeconomic factors, stress issues, etc.) that could impact successful outcomes (phenotypic changes, weight loss, etc.)
- Perform comprehensive patient history intake
- Perform required 4myheart data entry
- Engage in multi-disciplinary approach for improved patient outcomes to include their Regional Manager, physicians, territory sales managers, 4myheart program manager and other key personnel as needed
- Engage in multi-disciplinary approach to assist in sales objectives (i.e. test volume, client satisfaction with program, process improvement, etc.) to include CE regional managers, territory sales managers and other key personnel as needed
- Submit weekly reports as required by Regional Manager
- Participate in ongoing communication with Regional Manager regarding any issues associated with lipid clinic
- Provide educational programs and information for physician office personnel and community groups
- Contribute to process improvement by assessing potential deficits/strengths in patient education material and or internal support to 4myheart Center; communicate assessment to Regional Manager
- Maintain and develop corporate image and reputation
- Participate in meetings, conferences and project team activities
- Pursue personal development and knowledge necessary for the effective performance of the role.
- Understand physiologic and pharmaceutical implications for treating abnormalities identified through Berkeley testing
- Develop and maintain relationships with co-workers and support personnel within the Berkeley organization
Requirements:
- Minimum of Bachelor's degree in appropriate discipline, or graduate of an accredited bachelor or associate degree nursing program
- 5 years experience in professional certified role preferred
- Appropriate professionals would include nurses, exercise physiologists, and dietitians
- Cardiology/lipid management/patient education experience preferred
- Knowledge of pharmaceutical and non-pharmaceutical interventions for management of patients with dyslipidemia and/or other CV risk factors
- Excellent verbal, written and organizational skills required
- Self-motivated, results-oriented with a positive outlook
- Energetic personality that serves to motivate patients
- Mature, credible and comfortable interacting with physicians
- Must be willing to operate a variety of standard office equipment including computer and associated software programs, fax machine, photocopy machine and shredding machine
- Must possess excellent face-to-face, email and phone communication skills
- Some travel is necessary
- Flex hours may be required
